As some of you already know, I am writing to inform you that I will be stepping back from general practice to pursue special interest medicine. My last consulting day will be Friday 17th September 2021.
Your medical record will continue to be held securely at Jasper Medical. Either reception staff or I can arrange an introduction to another doctor for you to continue your care. I have attached a list of the doctors at the practice overleaf who are currently accepting patients.
I can also facilitate a summary of the important information in your file so that your new doctor has this material readily available at your first consultation. Please book an appointment with me to arrange this.

Making this decision has taken much consideration, and leaving a practice with so many wonderful patients will be difficult. I have highly valued the trusted relationship we have developed, and appreciated the opportunity to help you manage your health care needs, as well as support you during challenging times.
I may return to general practice in the future, so hopefully I will have the opportunity to see you all again. Thank you for the privilege of being your general practitioner, and I wish you the best of health.
